Under the direction of the primary care operations team, the Primary Care Nurse works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Nurse will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel. Position Details: Mercy Clinic Primary Care - Mercy Clinic Creve Coeur Location: 12855 N 40 Dr, Ste 280, Saint Louis, MO 63141 Schedule: Monday - Friday, 0800-1630 No nights, weekends, or holidays! **Must complete a minimum of three months of on-site training prior to transitioning to a hybrid work-from-home schedule. After training, this role follows a 50/50 hybrid model, with full-time (1.0 FTE) co-workers expected to work on-site five days per pay period. Must also be available to attend monthly on-site clinic meetings.** Our Primary Care Nurse is a newly created role for the clinics. In this position the nurse will work closely with the Providers and manage triage calls/messages for the clinic. They will have the option to move hybrid after training is complete. LPN Qualifications: Education: Diploma in Practical Nursing. Licensure: Active LPN license. Experience: 2-3 years of experience preferred. RN Qualifications: Education: Associate of Applied Science in Registered Nursing, Graduation from registered nursing program, bachelor's degree in nursing (BSN), OR 4 years of commensurate experience in lieu of the bachelor's degree. Licensure: Active RN license. Experience: 2-3 years of experience preferred.
